Individuals who are interested in Community Counseling Services will need to schedule an intake appointment at their local center. The intake fee is determined based on source of income and applicable insurance coverage. When contacting the appropriate county office to schedule an intake, the intake fee will be discussed at the time. Payment is expected at the time of service. For your intake appoint, please bring:
- Picture ID
- Proof of household income
- Social security card
- Insurance cards
- Proof of residence
- For individuals under the age of 18, birth certificate or proof of guardianship/custody papers
- List of current medications
- If coming from an inpatient facility, discharge paperwork from previous treating facility
